I’d like to take a moment and welcome you to Community Health Services (CHS), one of the area’s most respected primary health care delivery systems. Please feel free to browse our site and get to know us.

Community Health Services is a Federally Qualified Health Center that provides medical and dental care in 3 counties (Henry, Huron and Sandusky) from 7 locations. Our Board Certified Physicians provide care in Family Practice, Pediatrics, Podiatry, Internal Medicine, Obstetrics, Gynecology and Dentistry. CHS is voluntarily accredited by the Joint Commission on Accreditation of Health Care Organizations (The Joint Commission) and we have a proven track record of meeting or exceeding standards set for the health care industry.

In existence since 1970, our staff is dedicated to providing the highest standards of customer service at all of our locations. We pride ourselves with high patient satisfaction scores which are posted annually on this site.

No one seeking care at any of our offices will be denied service based on their source of payment. We gladly accept all Medicaid Managed Care Plans, Medicare, Insurance, and we offer a sliding fee scale based on a patient’s family size and income.
