1. What do I need to become a New Patient at Community Health Services? You first need to fill out our New Patient Packet. This can be downloaded from our
Forms page or can be acquired at the CHS office you would like to attend.
2. Do I need to make an appointment? Yes, our Providers see patients by appointment only.
3. What insurance/payment methods do you accept? We offer a sliding fee scale at Community Health Services. The application is available
here. You can also view the various insurance and payment methods we accept at our
Insurance Accepted page.
4. What type of payment is expected? Some type of payment is expected at the time of your visit if required. This varies from private insurance co-pays to self-payment on a sliding fee scale. Insurance co-pays/minimum payments are due at the time of your visit.
5. How do I transfer into or out of Community Health Services? In order to transfer your records to us or from our records department you need only to fill out a records release form that can be found
here.

7. How do I make an appointment? An appointment can be made by calling the office you frequent. Contact information can be found
here . Please give 24 hours notice when cancelling or rescheduling appointments.
8. When can I expect to have my prescription refilled? All refills on medications will be called to your pharmacy after 5:00 pm. Please do not call back to see if your refill has been called in. All prescriptions are called in within 24 hours.
9. How long will it take for the provider to complete my disability forms? It may take at least 30 days for the completion of disability forms as well as insurance forms and the actual transfer of records.
10. I was a patient 5 years ago, do I still need to fill out New Patient information? Yes, our policy is that once you have been away from our practices for over 3 years you must fill out a New Patient Packet and be seen as a new patient once again. You can find that information at our

11. Are minors able to receive treatment? In order to receive treatment, minors must be accompanied by their parent/guardian or an individual who receives prior written approval from their parent/guardian.
