Health and Dental Insurances Accepted:
The alphabetical lists below indicate which health and dental insurances are accepted at CHS offices. It does not guarantee that our providers are in your network. It is the responsibility of the patient to verify that their provider is in their network.
The Primary Care Provider (PCP) on your Medicaid card is at times just assigned. This does not mean that you are a patient of that PCP. You must still become a New Patient with Community Health Services if you have not been to our office before or within the last 3 years.
As a Federally Qualified Health Center we are able to accept any type of Medicaid.
