We are delighted that you have selected CHS to meet your health care needs. As a new patient, here's what you need to do prior to your first visit.
1. Complete your new patient packet
A new patient packet must be completed and returned to your CHS office prior to your first appointment.
You can access the secure online New Patient Packet here. This packet will be sent to CHS when submitted.
You can also pick up a new patient packet at any of our CHS offices, or download the packet below to print and complete.
2. Mail or drop off your completed new patient packet to any of our CHS locations
You can schedule your first appointment at that time, or a CHS representative will contact you promptly to schedule your appointment.
CHS accepts Medicaid, all Medicaid Managed Care plans, Medicare and most insurance plans. We offer a sliding fee schedule or discount of fees for qualifying patients. No patient is turned away from a CHS office based on financial status. For a complete list of insurance plans accepted or more information about payment options, please click here.
Note: If you're self-pay, you may be asked to complete a sliding fee scale application. All patients may apply for the sliding fee scale regardless of insurance status.
The CHS Patient Portal is a convenient and useful resource for you to access your health information and manage your health care activity.
You should receive an email from Follow My Health if you provided your current email address during your first appointment and completed your new patient packet. Please click on the blue “Patient Portal” icon at the top of this page for more information or to log into the patient portal.
